Co-ordinating comprehensive support

Case management

Dedicated expert support to identify and coordinate care when mental health, addiction, trauma, or life becomes too complex to manage alone.

What are the next steps & what to expect?

For moments when things feel unsafe, unmanageable, or impossible to hold alone

When things have reached crisis point, we help you work out what to do next - quickly, calmly, and safely.

If you or someone close to you is in a serious mental health, addiction, trauma, or behavioural health crisis, it can be hard to think clearly or identify the best of set options. We help you assess the situation, identify safe, tried and tested next steps, and put the right support around the person in crisis.

We look at the whole picture - not just the individual, but the family, support network, and immediate risks as well.

NB: If there is immediate danger or a medical emergency, call emergency services immediately.
Frequently asked questions
Can you refer me to a treatment centre / psychiatrist / psychologist?

We work with a variety of providers and professionals in the addiction and mental health field. We often work alongside these professionals to provide wrap-around care. If PBH is not the right service for you, or we feel we are not the right fit for you we will happily make a recommendation to another provider that may be able to help.

Does PBH follow a particular treatment approach or modality?

The way we support clients is flexible as we recognise that different people are at different stages in their road to wellness. We do not advocate or prescribe a particular modality but do recognise that clients who are engaged in self-destructive behaviours often need to be supported to stop or minimise the behaviour in order for their recovery to progress. However, we also acknowledge that this is not a linear process and as such lapses are frequent and we are adept at dealing with them.

How much does Case Management cost?

The prices for our services vary depending on what is required. Please get in touch to discuss your requirements and we can explain what PBH can provide and how much it will cost.

Do interventions work?

Interventions can and do work if conducted by professionals, are well planned and are reliant on friends, families and colleagues 'buying in' to the process. However, interventions are not guaranteed and the person who is being helped may reject help or may not engage successfully in treatment following and intervention. This is part of the risk of the process and will be explained to all involved as part of the process.

What to expect
Urgent practical joined up care plan, to include:
Immediate guidance on next steps
Help thinking clearly in a high pressure situation
Support for both the person in crisis and those around them
Fast access to appropriate professionals (where possible)
Help co-ordinating urgent assessments, treatment options and practical arrangements

Bringing structure and forward thinking to keep your care joined up

Structured, ongoing support to help bring professionals, plans, and people together keeping your care connected, clear, and moving forward.
‍
Mental health, addiction, trauma, and recovery support can become a reactive scramble - different clinicians, services and providers all moving independently with everyone responding to the latest crisis instead of following a plan. We bring organisation and structure to the moving parts, so support runs on a plan rather than a reaction.
‍
We work like an extended family member: someone who already knows your context and history, but can stay objective and stay focused on action. And we think in projects, not tasks - less "what's the next appointment" and more "where is this all heading, and what needs to happen to get there." That means formulating, tailoring, and updating treatment plans and goals as needs and circumstances change - and matching clients with the right professionals to make it happen.
‍
When you can't see the wood for the trees, we hand you the map - coordinating care, facilitating treatment, sharing information, and keeping plans moving, so support feels manageable again.

What to expect
Providing medium to long-term oversight of behavioural health care, including:
Identifying, making informed suggestions for and creating an appropriate behavioural/mental health clinical multi-disciplinary support team
Weekly progress reporting and monthly MDT meeting to review and  reassess care plan against agreed measurable goals and actions
Ongoing proactive co-ordination and management  of mental/behavioural health care providers and support network

Skilled help that’s ahead of what's coming, present for what's now

Coming out of an immediate crisis or from a period of time in hospital or treatment is when the real work starts - we understand that, and we know how vital it is to understand what "normal" looks like for each individual. We quickly establish that baseline, then keep revisiting it as things evolve. Rebuilding your life post-trauma, post-addiction, post-breakdown takes courage, and, most importantly, understanding.
‍
We work with individuals and their support networks to build a life beyond treatment, beyond recovery. We get to know you inside and out, so we're there in the moments you need someone - working with you to map a roadmap to a healthier, happier you, planning for triggers, and making sure your clinical and wraparound care keeps pace as your needs change and evolve.
‍
We work proactively, anticipating likely needs, risks, and transitions, and putting structure in place before problems escalate - combining a structured, proactive approach with a client-centred, relational focus, so clients can achieve the best possible outcome
